AVOID WEARING PERFUME, COLOGNE, BODY SPRAYS
24 HOUR NOTICE REQUIRED FOR CANCELLATIONS OR RESCHEDULING. I CAN REFUSE FUTURE APPOINTMENTS.
DO NOT HAVE A MASSAGE UNDER THE INFLUENCE OF ALCOHOL
TURN YOUR CELL PHONE OFF/SILENT (Don't ruin the massage by adding your own sound track)

Information regarding health issues is of vital importance such as medications, recent and chronic health conditions will determine how a massage is conducted.
Please inform us of conditions such as: Pregnancy, Cancer, Medications, Recent Surgeries, Recent Injuries, Recent Vehicle Accidents, All Muscle and Joint Issues, Allergies, ie.
Right of Refusal- I reserve the right to refuse service to anyone. This includes but is not limited to anyone who requests treatment or services that are outside of my scope of practice. I will exercise this right if anyone is under the influence of alcohol (intoxicated), recreational drugs, exhibits inappropraite behavior, or does not meet the standards of cleanliness. I reserve the right to charge for the session, whether or not the services were rendered, if I so choose.
